El Proyecto Fenix, Inc. (EPF), Emergency Assistance Program (EA), is designed to provide temporary limited emergency assistance to families that are currently registered with EPF. All applicants must meet the qualifications set by the Board of Directors (see below). Upon completion of the application, the Board will determine the type and amount of assistance to be provided on a case by case basis. All decisions of the Board are final.
REQUIREMENTS: 1 – Applicant understands the Emergency Assistance Requirements and Qualifications: A) Applicant is registered with EPF (has an application filed with EPF). B) Applicant(s) must adhere to and complete all the requirements of EPF EA program (and property owner(s) for rental assistance). C) All applications require EPF Board approval. D) Assistance is limited to a maximum of 90 days, on a case by case evaluation of need, qualifications, and Board recommendation and approval. E) Applicant(s) agree to adhere to and complete all the requirements of EPF EA (Program and Property owner(s) for rental assistance). F) EPF EA program requires reimbursement to the EPF emergency fund based on the recommendation of EPF Board. G) Applicant must submit their plan for self-care after the assistance period (90 days maximum). H) EPF is not responsible to continue assistance under the following circumstances: I. Recipient does not adhere to agreement with EPF Board recommendation(s) II. The 90-day term is reached III. Applicant asks to terminate this Agreement (in writing)
2 – EPF Requires: A) A copy of the Recipient’s contract/lease/rental agreement for rental assistance. B) Signed Emergency Assistance Agreement C) Property owner’s phone number and property address
